I understand that Partners in Home Care Services (PIHCS) will provide the curriculum for my 60-hour online coursework and 16-hour in-person skills lab as part of the Certified Home Health Aide (CHHA) training program.

I acknowledge that the Proof of Employment (POE) is a mandatory requirement set by the New Jersey Division of Consumer Affairs Board of Nursing (NJDCA BON) for obtaining a CHHA license.

The POE serves as verification that CHHA applicants have secured employment or a job offer from a legally operating home care agency.

I further acknowledge that PIHCS does not issue state licenses, but is solely responsible for providing the training certification required for me to apply for my CHHA license through the NJDCA BON.

Additionally, I understand that PIHCS will provide guidance on the licensing process, and I agree to follow all necessary steps to obtain my state certification.

I also acknowledge that as part of the CHHA licensing process, the New Jersey Board of Nursing will conduct a criminal background check, which is a mandatory requirement for all applicants.
